We are looking for a<strong> Field Care Specialist I</strong></span><em><span style="font-weight: 400;">&nbsp;</span></em><span style="font-weight: 400;">for our team in <strong>Missouri</strong>.</span></p> <p><strong><em>This role requires frequent travel throughout the St. Charles, MO area, with potential to travel to our FreedomCare office.</em></strong></p> <p><strong>Department &amp; Position Overview:</strong></p> <p>Working closely with patients, caregivers, and departments across the business, you must be able to provide seamless Customer Service experience, while ensuring that our clients are receiving the care they need. We will nurture your talent and provide continued career development opportunities, from your induction and throughout your career with us.</p> <p><strong><span data-contrast="auto">Every Day You Will:</span></strong><span data-ccp-props="{"201341983":0,"335559738":240,"335559739":240,"335559740":259}">&nbsp;</span></p> <ul> <li>Manage an assigned patient's caseload</li> <li>Adhere to a revolving monthly patient's service and compliance standard</li> <li>Direct point-of-contact for enrolled member questions, inquiries or concerns</li> <li>Experience with data input, case review and building rapport and confidence patients and caregivers</li> <li>Adept at multitasking, excellent communication (written and verbal) and organization skills</li> <li>Collaborate with Care Coordinators in scheduling home visits using tools available for efficiency</li> <li>Conduct home visits and complete necessary documentation to meet compliance requirements</li> <li>Review results of home visits with Care Coordinators and create follow-up steps, if necessary</li> </ul> <p><strong><span data-contrast="auto">Ideal Candidate Will Possess:</span></strong><span data-ccp-props="{"201341983":0,"335559738":240,"335559739":240,"335559740":259}">&nbsp;</span></p> <ul> <li>1+ years of experience in field-based roles</li> <li>Capable of assessing patient health risks and identifying potential hazards within the home environment</li> <li>Motivation to meet and/or exceed case management targets or metrics</li> <li>Excellent written and verbal communication skills</li> <li>Excellent time management, critical thinking, and active listening skills; adept at multitasking</li> <li>Must have poise and patience when dealing with customers</li> <li>Demonstrates a high level of empathy and emotional intelligence</li> <li>You must have your own reliable transportation an
