This role combines clinical assessment, care coordination, patient and Personal Care Attendant (PCA) education, and quality assurance to ensure safe, effective, and person-centered care.</span><span data-ccp-props="{"201341983":0,"335559739":60,"335559740":276}">&nbsp;</span></p> <p><span data-ccp-props="{"201341983":0,"335559739":60,"335559740":276}">&nbsp;</span><span data-contrast="auto">The Case Manager, RN collaborates closely with patients, PCAs , and interdisciplinary care teams to assess needs, develop individualized plans of care, and facilitate the seamless initiation and delivery of services. Through regularly scheduled home visits, the RN evaluates patient and caregiver well-being, monitors the quality and effectiveness of care, and identifies opportunities to enhance health outcomes, safety, and regulatory compliance.
