This is a non-clinical role focused on building referral relationships, connecting clients and families with home health services, and supporting successful transitions from hospitals and other health care settings to care at home.</p> <p> </p> <p>At BAYADA, our mission is to help people have a safe home life with comfort, independence, and dignity. Our work is guided by The BAYADA Way: compassion, excellence, and reliability.</p> <p><strong>What you’ll do</strong></p> <ul> <li>Develop and maintain strong professional relationships, in accordance with The BAYADA Way, with our referral partners and their teams.</li> <li>Serve as a knowledgeable and trusted resource on home health services and the transition from facility-based care to care at home, <em>and when a referral to home health directly from a home visit would positively impact outcomes and patient experience </em><em>recognizing that many referrals will come from NPs whose patients are already living at home.</em></li> <li>Clearly articulate the features and benefits of BAYADA services to referral partners, clients, and families.</li> <li>Identify individuals who may benefit from home health care and coordinate appropriate referrals with the BAYADA service office.</li> <li>Develop a strong understanding of BAYADA’s home health services, referral process, and available resources.</li> <li>Gather and communicate referral information accurately to the appropriate BAYADA service-office and clinical teams.</li> <li>Coordinate the non-clinical aspects of the referral process and support timely communication among referral partners, clients, families, and BAYADA teams.</li> <li>Follow up with referral sources and internal partners to help ensure a smooth transition of care.</li> <li>Maintain accurate referral information, follow-up activity, and required documentation.</li> <li>Participate in market development activities and support growth through ethical, service-focused relationship building.</li> <li>Represent BAYADA professionally in hospitals, rehabilitation facilities, physician practices, community settings, and other referral environments throughout the ERM market.</li> <li>Collaborate with Transitional Care Managers, Marketing Managers, clinical leaders, and service-office teams to support clients and referral partners.</li> </ul> <p><strong>Qualifications</strong></p> <ul> <li>This is a non-clinical position; a clinical license is not required.</li> <li>At least three years of experience in sales, marketing, community relations, health care business development, referral coordination, customer service, or a similar role.</li> <li>Previous experience working with hospitals, rehabilitation facilities, physician practices, discharge planners, or community-based health care partners preferred.</li> <li>Demonstrated record of strong interpersonal, communication, and relationship-building skills.</li> <li>Ability to build credibility and establish productive relationships with referral sources and hea
