This role supports the Intake team ensuring the patient and caregiver in providing education, submitting required documents, and brief orientation to ensure coordination for start of care.</strong></p> <p><strong><span data-ccp-props="{"335557856":16777215,"335559738":220,"335559739":220}">Every day you will:&nbsp;</span></strong><br><br><span data-ccp-props="{"335557856":16777215,"335559738":220,"335559739":220}">· Visit patients and caregivers in their homes to support the enrollment process to start of care. </span><br><span data-ccp-props="{"335557856":16777215,"335559738":220,"335559739":220}">· Act as primary point of contact supporting the completion of eligibility and employment paperwork for the caregiver.&nbsp;</span><br><span data-ccp-props="{"335557856":16777215,"335559738":220,"335559739":220}">· Demonstrate effective case management by maintaining a 3-hour email, chatter and voicemail response time.&nbsp;</span><br><span data-ccp-props="{"335557856":16777215,"335559738":220,"335559739":220}">· Achieve monthly standard to enroll a minimum 40% of caregivers based on state standards. </span><br><span data-ccp-props="{"335557856":16777215,"335559738":220,"335559739":220}">· Navigate multiple systems as required to complete the enrollment process. </span><br><span data-ccp-props="{"335557856":16777215,"335559738":220,"335559739":220}">· Maintain detailed case notes in Salesforce. </span><br><span data-ccp-props="{"335557856":16777215,"335559738":220,"335559739":220}">· Adhere to HIPAA guide
