Transition Team - Registered Nurse (Private Duty)

The role is FT – 40 hours per week. This team will support the transition of care from the hospital team to ongoing home care services. This role offers a unique opportunity to learn how to care for clients with rare and complex diseases.&nbsp; As part of the Transition Team, you will be responsible for delivering quality care to clients and providing extensive training to the ongoing care team.</strong></div> <div class="x_elementToProof" data-ogsc="rgb(0, 0, 0)">&nbsp;</div> <div class="x_elementToProof" data-ogsc="rgb(0, 0, 0)"><strong data-ogsc=""><em data-ogsc="">Present openings are day shifts (Monday through Sunday) and overnight shifts (Monday through Sunday).
