We are looking for a<strong> Care Manager </strong>for our team in Indiana<strong>.</strong></span></p> <p><strong><em>This is a field-based position that requires frequent travel to patient's homes in the Terre Haute, IN area.</em></strong></p> <p><strong>Program &amp; Position Overview:</strong></p> <p><span data-contrast="auto"><span class="TextRun SCXW119221864 BCX0" lang="EN-US" data-contrast="auto"><span class="NormalTextRun SCXW119221864 BCX0">The Care Manager </span><span class="NormalTextRun SCXW119221864 BCX0">will be a vital part of our operations in </span><span class="NormalTextRun SCXW119221864 BCX0">Indiana</span><span class="NormalTextRun SCXW119221864 BCX0">. </span><span class="NormalTextRun SCXW119221864 BCX0">Responsibilities include performing orientations, providing an assessment of new patients to our program, and training our caregivers to ensure they provide the best service possible to their loved ones.</span></span><span class="EOP SCXW119221864 BCX0" data-ccp-props="{}">&nbsp;</span></span></p> <p><strong>Every Day You Will:</strong></p> <ul> <li style="font-weight: 400;"><span style="font-weight: 400;">Evaluate the home care needs of the client&nbsp;&nbsp;</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Assess the environment&nbsp;</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Fitness of the caregiver to provide the care&nbsp;</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Home orientations involving signing of paperwork</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Initial assessments during orientations</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Periodic supervisory visits / check in for on-care cases</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Training caregivers / patients as required per case</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Interacting with the care manager for the patient as needed</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Engaging in interdisciplinary meetings with the care managers, caregivers and patients</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Collaborate in care and communicate impressions of cases to appropriate staff</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Critically assess procedures and implement improvements for evaluation of cases, feedback to appropriate parties, and training of caregivers</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Wear multiple hats as operations develop</span></li> <li style="font-weight: 400;"><span style="font-weight: 400;">Travel within the state of Indiana (reimbursement for mileage and expenses)</span></li> </ul> <p><strong>Ideal C
