We are looking for a <strong>Registered Nurse (RN) for our team in Missouri.</strong></p> <p><em><strong>This is a hybrid role that requires&nbsp;<span style="text-decoration: underline;">fieldwork</span> to visit patients’ homes in Kansas City, MO, and surrounding areas, with the possibility of occasional commuting to our Kansas City, MO office.</strong></em></p> <p><strong><span data-contrast="auto">Department &amp; Position Overview:</span></strong><span data-ccp-props="{"201341983":0,"335559739":160,"335559740":259}">&nbsp;</span></p> <p><span data-contrast="none">Personal Care Services is a caregiving arrangement in which a patient has a principal caregiver who provides daily support based on the patient's daily care needs. The goal of this service is to provide necessary care while fostering and emphasizing the patient’s independence in the patient’s home that will provide them with a range of care options as their needs evolve. The service&nbsp;is designed&nbsp;to provide options for alternative long-term care to&nbsp;persons&nbsp;who meet nursing facility-level care needs while preserving the dignity, self-respect, and privacy of the patient. </span><span data-ccp-props="{}">&nbsp;</span></p> <p><span data-contrast="auto">In this position,&nbsp;as a&nbsp;licensed practical nurse, you&nbsp;will perform clinical evaluations in the home of the&nbsp;Personal Care Services&nbsp;member to&nbsp;determine&nbsp;the level of service to qualify for the program.&nbsp;&nbsp;You&nbsp;will also&nbsp;conduct monthly visits&nbsp;with&nbsp;members who are On Care with&nbsp;FreedomCare&nbsp;to ensure they are receiving quality service&nbsp;and address any changes in their condition.</span><span data-ccp-props="{}">&nbsp;</span></p> <p><strong>Every Day You Will:</strong></p> <ul> <li><span data-contrast="none">Provides support to the caregiver and member, and reviews services according to scope of the Personal Care Services program.</span></li> <li><span data-contrast="none">Respects client and employee rights and privacy, ensure the security of protected information, practices in an ethical manner and is compliant to the agency’s compliance program and privacy policy.</span></li> <li>Follows the FreedomCare’s clinical policy and procedures.</li> <li>Assesses and regularly reassess the client’s needs and their PCP summary form, treatment and education and communicates any changes to the member’s PCP.</li> <li>Develops implements, evaluates, revises, and provides input to the client’s plan of care to include the physician’s plan of treatment, and problem list. Provides instruction of care to Caregiver according to the plan of care.</li> <li>Completes all client documentation, to include written records, reports, and notes in accordance with agency policy and procedure following client visits or contacts in an accurate, timely, complete, concise and legible manner.</li> <li>Actively coordinates the care of the client through communication and written documentation to the physician, RN and Program Director, other disciplines involved in the client’s care and community resources.</li> <l
