You will build trusted relationships within a large hospital campus, serve as a knowledgeable clinical resource, and help connect patients and referral partners with high-quality care and support.</p> <h2 class="wnfdnti _1ibi0s33y _1ibi0s34u">The Opportunity</h2> <p class="wnfdntu _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">The Transitional Care Manager will be highly visible and active within the hospital environment. This role is ideal for an energetic Registered Nurse who enjoys connecting with people, building relationships, and becoming a trusted resource for case managers and other key partners.</p> <p class="wnfdntu _1ibi0s3f5 _1ibi0s3ce _1ibi0s3ea">You will spend the majority of your time on-site, developing strong relationships with hospital staff and referral partners.
