This is a non-clinical role focused on building referral relationships, connecting clients and families with home health services, and supporting successful transitions from hospitals and other health care settings to care at home.</p> <p>At BAYADA, our mission is to help people have a safe home life with comfort, independence, and dignity. Our work is guided by The BAYADA Way: compassion, excellence, and reliability.</p> <p> </p> <p><strong>What you’ll do</strong></p> <ul> <li>Develop and maintain strong professional relationships, in accordance with The BAYADA Way, with <strong>Ennoble Care stakeholders and partners directly connected to the Ennoble partnership and referral pathway.</strong></li> <li>Serve as a central point of coordination for the <strong>two-way Ennoble–BAYADA referral pathway</strong>, supporting referrals from Ennoble to BAYADA home health as well as referrals from BAYADA to Ennoble Care.</li> <li><strong>Drive referral conversion and timely follow-through</strong> by monitoring referrals through the scheduling and care-initiation process, identifying barriers or delays, and coordinating with the appropriate Ennoble and BAYADA teams to help move referrals forward.</li> <li><strong>Manage and review referral pending reports daily</strong>, proactively following up on outstanding referrals, escalating barriers when appropriate, and helping ensure referrals are scheduled, completed, or otherwise appropriately resolved in a timely manner.</li> <li>Help maintain visibility into referral status across the partnership and <strong>close the communication loop</strong> with appropriate Ennoble and BAYADA stakeholders when referrals are delayed, unable to be scheduled, or require additional action.</li> <li>Build trust and engagement with <strong>internal Ennoble champions and external partners involved in supporting the Ennoble partnership</strong>, while maintaining clear alignment with BAYADA’s local sales teams and established referral-source relationships.</li> <li>Develop a strong understanding of BAYADA’s home health services, Ennoble’s care model, the Ennoble–BAYADA referral processes, and available resources in order to effectively support both sides of the partnership.</li> <li>Identify opportunities to strengthen the <strong>Ennoble–BAYADA referral pathway</strong> by recognizing trends, identifying operational barriers, facilitating solutions, and supporting consistent processes across markets.</li> <li>Gather and communicate referral information accurately to the appropriate BAYADA and Ennoble teams.</li> <li>Coordinate the non-clinical aspects of referrals and facilitate timely communication among Ennoble teams, BAYADA service offices, clinical teams, clients, and families as appropriate.</li> <li>Maintain accurate referral information, follow-up activity, pending status, and required documentation related to the Ennoble partnership.</li> <li>Partner closely with <strong>BAYADA local sales leadership and assigned marketers</strong>, ensuring Enn
