This role collaborates closely with the <strong>Director, Service Supervisor, Client Services Manager, and office team</strong> to ensure safe, high-quality, compassionate care while maintaining compliance with BAYADA policies, clinical standards, and regulatory requirements.</p> <p>The Clinical Manager serves as a clinical resource for field staff, conducts client assessments, develops and oversees individualized care plans, and supports positive clinical outcomes through effective care coordination. In partnership with the Service Supervisor, this role contributes to caregiver support and development, client satisfaction, workforce stability, and the achievement of BAYADA's mission and Assistive Care operational goals.</p> <hr class="wnfdnts _1ibi0s34s _1ibi0s342 _1ibi0s390 _1ibi0s38a _1ibi0s34 _1ibi0s3cu _1ibi0s312"> <h2>Key Responsibilities</h2> <h3>Clinical Leadership &amp; Client Care</h3> <ul> <li> <p class="wnfdntf _1ibi0s3e6 _1ibi0s3ce _1ibi0s3db">Conduct comprehensive client assessments in the home and complete the care planning process, including initial and ongoing reassessments as required by policy and payor regulations.</p> </li> <li> <p class="wnfdntf _1ibi0s3e6 _1ibi0s3ce _1ibi0s3db">Develop, implement, and update individualized care plans that reflect client goals, functional status, risks, and social determinants of health; ensure alignment between written and electronic care plans.</p> </li> <li> <p class="wnfdntf _1ibi0s3e6 _1ibi0s3ce _1ibi0s3db">Provide ongoing clinical and case management oversight for an assigned caseload, ensuring services are delivered safely, effectively, and in accordance with BAYADA standards and state/federal regulations.</p> </li> <li> <p class="wnfdntf _1ibi0s3e6 _1ibi0s3ce _1ibi0s3db">Collaborate with physicians and other health care providers to coordinate care, obtain and document verbal orders, and ensure timely, accurate completion of required clinical documentation and orders.</p> </li> <li> <p class="wnfdntf _1ibi0s3e6 _1ibi0s3ce _1ibi0s3db">Conduct clinical visits (reassessments and supervisory visits) at required frequencies, documenting changes in condition, risks, and interventions; escalate complex cases to clinical leadership as appropriate.</p> </li> </ul> <h3>Caregiver Oversight, Development &amp; Engagement</h3> <ul> <li> <p>Provide clinical supervision, guidance, and professional development for home health aides and other field employees, including skills assessments, competency evaluations, and ongoing education in both home and office settings.
