<p style="text-align: left;"><strong>***$10,000 Sign-on Bonus***</strong></p> <p style="text-align: left;"><strong>When you know Home is the future of Healthcare</strong></p> <p style="text-align: left;">Jefferson Health at Home by BAYADA was founded on the principle that healthcare gets better when people get better care at home – the place where they feel best. &nbsp;</p> <p style="text-align: left;">We are currently recruiting for a compassionate<strong> Hospice Registered Nurse (RN) Case Manager </strong>to reliably serve patients in small territories throughout&nbsp;<strong>Montgomery County and Bucks County, PA.</strong></p> <p style="text-align: left;"><strong>What you’ll do: </strong></p> <p style="text-align: left;">As a Jefferson Health at Home by BAYADA Hospice Nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, affording them the opportunity to receive the medical care required to remain at home.&nbsp; You will:</p> <ul style="text-align: left;"> <li>Making home visits to clients in designated geographic territories.</li> <li>Performing assigned duties, including administration of medication, wound care, treatments, and procedures and comfort care.</li> <li>Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.</li> <li>Following up with, executing, and properly documenting doctors' orders.</li> <li>Occasional on call after hours and/or weekends</li> </ul> <p style="text-align: left;"><iframe src="https://www.youtube.com/embed/lpkoHoK9pCo" width="560" height="314"></iframe></p> <p style="text-align: left;"><strong>What you should have:</strong></p> <ul> <li>Active and current license to practice as a professional registered nurse (RN) in the state of PA.</li> <li>One year of hospice experience strongly preferred.</li> <li>Previous home healthcare experience welcomed, but not required.&nbsp;</li> <li>Ability to travel to cases as assigned</li> <li>A calm and reassuring presence, active listening skills, excellent assessment skills and strong written and verbal communication.</li> <li>The ability to deliver patient care as you would want those you love to be treated, with compassion, excellence, and reliability<em>.</em></li> </ul> <p style="text-align: left;"><strong>Why you’ll love what you do at Jefferson Health at Home by BAYADA:</strong></p> <p style="text-align: left;">We are committed to providing patients the highest-quality care, enabling them to live at home with comfort, independence, and dignity.
